To schedule an appointment, please call our office at 812-376-3311. Please explain the reason for your visit. This allows us to schedule the appropriate time needed to address your healthcare concerns. For your first visit, please arrive 15 minutes prior to your appointment time so that we may obtain the information required for establishing your care. 

You play a major role in helping us stay on schedule. If you are running late, please phone before you come, as we may ask you to reschedule your appointment so that our other patients are not inconvenienced. If you arrive late for your appointment, we will need to reschedule.

If you have an urgent need to be seen, please call as soon as possible, and we will try to accommodate you. We do our best to stay on schedule, but there are occasions when deliveries, emergency surgeries and physician illness can cause unexpected delays. We do our best to keep delays to a minimum and apologize for any inconvenience.

 Each visit, please:

• Bring your insurance card with you as well as a list of your current medications.
• Inform the front desk staff of any change in name, address, telephone number, employment or insurance.
• Inform your physician of any changes in medical history.

Please extend these courtesies to others in our office: no smoking, no heavy perfumes. Please turn off your cell phone when in the exam room.

Mammograms

If you are due for a mammogram and would like to discuss it with your doctor during your office visit, please schedule an appointment about a week prior to your ob-gyn office visit. Please call the Columbus Regional Hospital Breast Health Center (located at 2326 18th Street, Suite 110) at 812-376-5064 to make an appointment. Please note that a doctor's order may be needed for a mammogram. Call our office so we can send an order to the CRH Breast Health Center.

In-Network Health Plans

Your medical records are strictly confidential. The Health Insurance Portability and Accountability Act (HIPAA) restricts us from releasing any information without your written permission.

There may be times when you request that we provide copies of our records to you or other entities. You will be billed for this service.

You will receive prompt notification of any results that require immediate attention, otherwise please allow 7-10 days for test completion. For normal test results requiring minimal follow-up, we will notify you by mail. Otherwise our staff will call you with the results and the physician’s instructions.

If at any time you have additional questions, feel free to make an appointment with the doctor. If you do not receive notification of your results in the above time frame, please call the office to inquire about them.

Prescription refills will only be authorized during normal office hours. Allow 48 hours for this process to be completed. Please evaluate your medication supply prior to your office visits and request refills at your visit.

If you have not seen your doctor within the recommended time frame for follow-up, you will need an appointment before refills can be authorized. It is our policy that no controlled substances will be called in after hours or on weekends.

If you require medical attention after the office is closed, you can reach the doctor on call through the Columbus Regional Hospital operator at (812) 379-4441.

Please do not call after hours for:

• Test results.

• Appointment needs.

• Medication change requests, unless you are having a reaction.

If you have a true medical emergency, call 911 or go to the nearest emergency room.
