Employer Health Partners

Welcome!

Employer Health Partners is a practice managed through a collaborative relationship with Columbus Regional Health and select employers who have contracted with CRH to offer this dedicated employee healthcare solution to serve their employees and family members.

Employer Health Partners is available for use by any employee or dependent covered under the contracted employer's health insurance. All covered patients will be able to receive healthcare services in the clinic with no co-pay, and the the employer saves on healthcare costs through this dedicated, lower-cost service.

There is NO CO-PAY for visits, labs or medications provided within the clinic. Services in the clinic are paid for by their employer at a discounted cost, allowing both you and your place of business to save money on healthcare costs.

The clinic is located at NexusPark, 2100 25th Street, Suite I, Columbus, IN. Please use Entrance 3 for your convenience.

Appointment Scheduling

How do I make an appointment?

If you have never used the clinic before and this is your first visit, please call the clinic to set up your appointment. You can then set up a MyChart account to speed up your visit and complete your registration. After your first appointment ALL scheduling will be available through MyChart online 24/7!

Need to schedule a child? To protect all patients under the age of 18, you must create their profile in the clinic. Once you and your child have a MyChart account, you will be able to schedule all future appointments online.

Please bring your health insurance card with you to each visit.

The MyChart application is a platform you can use to access your medical record, keep track of healthcare services and appointments, schedule appointments, view test results, and even communicate with your physician. MyChart is available via the Internet and as a smartphone application. Although patient medical records will automatically be housed on the new Epic platform, each patient must sign up to use the MyChart application.
